Stabilization Program: The goal of Wellspring’s Crisis
Stabilization Program is to assist adults who are experiencing an
acute psychiatric episode. With our 24/7 care, we help people avoid
hospitalization, achieve stabilization, and recover from acute
mental health symptoms such as severe depression, mania, or
psychosis. Clients receive intensive treatment, close monitoring
with high levels of staff support, and medications prescribed by a
psychiatrist, if necessary, in a homelike setting. The 16-bed
program has two locations – the Samuel B. Todd Center and the David
J. Block Center. Both are licensed, accredited crisis stabilization
offering the only services of their kind in the Louisville Metro
region. The Wellspring Crisis Stabilization Unit (CSU) Targeted
Case Manager (TCM): The CSU TCM provides case management to adults
with severe and persistent mental illnesses who are homeless and
receiving services in the CSU. CSU TCM Duties and Responsibilities:
Assist clients in gaining access to medical, social, educational
and other support services. Develops and facilitates the pursuit of
individualized recovery goals via assessment, connecting to
resources and monitoring progress. Assisting clients in pursuing
housing options. Supervision of clients' self-administered
medications (SSAM), assisting clients with their engagement in
physical and mental health and addiction treatment, and ensuring
uninterrupted provision of all services. Models and assists clients
with illness management and independent living skills and provides
crisis intervention as needed. Participates with the
multidisciplinary treatment team as well as other providers and
sources of support. Travels throughout the community conducting
home visits and connecting clients with resources and services.
Collaborates with landlords and other community partners,
advocating for client needs and rights. Assists clients in
locating, applying for, and moving into housing. Assists clients
with illness management, medication management, and independent
living skills, providing crisis intervention as needed. Assisting
clients with their engagement in physical, mental health, and
addiction treatment, ensuring uninterrupted provision of care.
Completes electronic documentation of services demonstrating
medical necessity within 24 hours of client contact. The ideal CSU
TCM candidate will have a belief in recovery from mental illness
and an enthusiasm for helping persons with disabilities, rooted in
empathy and compassion and demonstrating a careful and caring
attitude. . ACT TCM Qualifications and Requirements: A personal
vehicle is required as the ACT TCM will be responsible for
traveling between work sites and transporting clients to regular
appointments. KY TCM Training BA or BS in field recognized by
DBHDID for TCM: psychology, social work or related field and at
least one year post Bachelor's working full-time with adults with
serious mental illness OR Master's Degree in related field. Salary
/ Other Benefits: $45,000 - $50,000 Compensation for on-call
