Job Description:
About the Role Care coordination is an integral part of
comprehensive cancer care that has had a positive impact on how
care is delivered to patients. With direction and guidance from the
Nurse Navigator, the Patient Care Coordinator role facilitates
communication within the interdisciplinary team and the patient to
proactively provide individualized, culturally sensitive care that
allows patients to have access to quality healthcare through all
phases of healthcare experience. What You’ll Be Doing Coordinates
and completes the utilization of genomics under the direction of
physician and Nurse Navigator Works under direct supervision and in
tandem with the Nurse Navigator to coordinate patient care
Initiates care coordination services upon diagnosis and continue
throughout all aspects of patient care, including survivorship
Identifies challenges and assists with potential solutions for any
barriers to care. Consideration will be given to the patient’s
values and their cultural, spiritual and social needs Under
direction of the Nurse Navigator, coordinates appointment needs and
schedules appropriately to help expedite the plan of care for the
patient Appropriately documents all patient interactions and
services provided, as required, within the electronic medical
record Acts as a liaison and advocate for patients, families and
caregivers when assisting the multidisciplinary team and community
resources Participates in continuous process improvement of patient
outcomes, assuring appropriate utilization of resources to increase
patient, family and caregiver satisfaction Serves as a resource
person for internal and external health care providers If and when
all duties are completed you are able to cross train with MA.

Education and/or Experience High school diploma Certified Medical
Assistant (experience working in healthcare setting preferred)
Language Skills Ability to read and interpret documents such as
safety rules, operating and maintenance instructions, and procedure
manuals. Ability to write routine correspondence. Ability to speak
effectively before individuals or groups of people. Mathematical
Skills Ability to calculate basic figures and amounts. Ability to
apply concepts of basic algebra and geometry. Reasoning Ability
Ability to solve practical problems and deal with a variety of
concrete variables in situations where only limited standardization
exists. Ability to interpret a variety of instructions furnished in
written, oral, diagram, or schedule form. Computer Skills To
perform this job successfully, an individual should have knowledge
of basic computer software. Other Skills and Abilities Knowledge of
medical terminology to communicate with clinical staff Knowledge of
health insurance and prior authorization process Knowledge of
common safety hazards and precautions to establish a safe working
environment Adapts easily to various environments Ability to read
and comprehend oral and written instructions Ability to follow
written protocols, policies and procedures Ability to establish and
maintain effective working relationships with the patients,
families, employees and the public Ability to communicate clearly
and effectively Ability to organize, track and maintain quality

Position Type/Expected Hours of Work This is a full-time position.
Days and hours of work are Monday through Friday, approximately 8
hours per workday. Occasional evening and weekend work may be
