Care & Preventive Health Deliver comprehensive primary care
services, including preventive care exams, preventive health
screenings, and risk assessments to optimize employee health
Diagnose, treat, and manage chronic conditions, using an
evidence-based, longitudinal care approach Address acute health
concerns, such as respiratory infections, gastrointestinal issues,
minor dermatologic conditions, headaches, and musculoskeletal
complaints Provide basic gynecologic care, including contraceptive
counseling, menstrual and menopause health management, and routine
pelvic/breast exams Administer and counsel patients on routine and
age-based vaccinations Educate employees on health behaviors,
self-care strategies, and disease prevention, helping them take an
active role in their well-being Care Coordination & Clinical
Excellence Engage in evidence-based primary care medicine services
while collaborating with the onsite physician for complex and
high-risk cases Serve as a trusted health resource for employees,
ensuring continuity of care and facilitating connections to
internal and external specialists as needed Apply data-driven
insights to support patient care planning, including reviewing
annual labs, biometric data, and wearables to personalize
interventions Patient Advocacy & Wellbeing Participate in
continuous quality improvement initiatives, finding opportunities
to enhance care efficiency and patient experience Collaborate with
corporate well-being teams to integrate preventive health programs
into broader employee wellness strategies Serve as a clinical
advocate for health equity, ensuring that care is inclusive,
culturally competent, and accessible to all employees Operational &
Administrative Responsibilities Maintain accurate, compliant, and
confidential medical records in the electronic health record (EHR)
system Ensure adherence to privacy, ADA, OSHA, and other workplace
health regulations as they pertain to primary care services Provide
on-call support for evening and weekend operations as needed to
maintain continuity of care Other duties within the scope of
practice as assigned by the supervising physician Basic
Qualifications Graduate of an accredited Nurse Practitioner (NP) or
Physician Assistant (PA) program Master’s degree in Nursing or
Physician Assistant Studies from an accredited university Active
certification as an Adult or Family Nurse Practitioner, Advanced
Practice Nurse, or Physician Assistant Minimum of 5 years of
clinical experience in primary care, internal medicine, family
medicine, or direct primary care models Additional Preferences
Current, unrestricted Indiana license as an Advanced Practice Nurse
(APN) or Physician Assistant (PA), including active
state-controlled substance registration (CSR) and DEA licensure
with prescriptive authority Willingness to obtain medical licensure
in additional states as needed to support employees across multiple
locations Qualified applicants must be authorized to work in the
United States on a full-time basis. Lilly will not provide support

Clinical experience in primary care, internal medicine, ambulatory
care, or emergency medicine. Certification in Advanced Cardiac Life
Support (ACLS) (or willingness to obtain) Strong knowledge of
electronic health records (EHRs), data analytics, and health
technology platforms APP will follow the provided approved Lilly
Policy and Procedures. APP will have on site only those approved
medications as outlined in Lilly standing orders Exhibit a high

on a candidate’s education, experience, skills, and geographic
location. The anticipated wage for this position is $127,500 -
$187,000 Full-time equivalent employees also will be eligible for a
company bonus (depending, in part, on company and individual
performance). In addition, Lilly offers a comprehensive benefit

eligibility for medical, dental, vision and prescription drug
