May be required to perform admission, routine, respite, or continuous care visits and assess patients within regulatory requirements for hospice. Responsible for overall coordination of the care delivered to her/his patient caseload. This is accomplished by making regular visits to patients wherever they reside, communicating regularly with the Interdisciplinary Team and Hospice Medical Director, identifying patient, family, caregiver needs and providing those needs in accordance with the attending physician's orders and the IDG plan of care. This position requires the ability to build rapport and communicate effectively with patients, families, physicians, facilities, and other IDG team members. In this role you will be responsible for: Completing initial evaluation and re-evaluating the patient's nursing needs during each visit. Assuming primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the patient plan of care. Completes assessments specific to the provision of Hospice care, including but not limited to identification of home safety risks, appropriateness of home environment to the delivery of care, availability and capacity of support systems and community resources. Provide appropriate support at the time of death and perform bereavement assessment if requested. Provide positive, supportive communication to physicians, patients, families, facilities, visitors, and other organization personnel. Required Qualifications Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing Current State License as a Registered Nurse Minimum 1 year of experience as a nurse in a clinical care setting Nursing skills as defined by accepted nursing standards Demonstrates ability to assess and respond to the needs of patients, families, and caregivers in varied settings and to function effectively in the home or nursing facility environment Demonstrates skill in nursing practice, ability to cope with ill, disabled, terminal patients, family emotional stress, and tolerance of individual lifestyles Demonstrates ability to handle multiple priorities, documentation requirements, and deadlines. Reliable transportation and valid automobile insurance coverage Desired Qualifications BSN preferred Hospice or home health experience preferred Location Office Location: 600 E. Carmel Dr, Suite 144, Carmel, IN 46032 Main Service Area: Carmel and surrounding Hours Office Hours: 8:00 am – 5:00 pm, Monday through Friday Weekend/On-Call Rotation Pay Range $80,000—$86,000 USD Background Screening Optimal Care conducts a background screening upon acceptance of a contingent job offer.
