the Position TravelNurseSource is working with Cynet Health to find
a qualified Home Health RN in Dublin, Ohio, 43017! Job Title:
Registered Nurse Profession: Nursing Specialty: Home Health
Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 -
17:00) Experience: Minimum 1 full year of Adult Home Health
Experience or 1 full year of Hospice experience depending on the
specialty License: Must have license in hand Certifications: CPR,
BLS Must-Have: - Proficiency in OASIS - Proficiency in HomeCare
HomeBase EMR within the last 6 months Description: Provides and
directs the provision of home nursing care according to physician
orders based on agency policies and procedures. Uses the competent
application of the nursing process. Responsible to Clinical
Manager. Directly supervises and evaluates Home Health Aides and
Licensed Practical Nurses/Licensed Vocational Nurses. Home visits
are required. Periodic office meetings will occur. Must have
reliable transportation, a current driver's license, and
agency-required liability insurance. Average number of patients per
day is 5-8. Each type of visit and activity (documentation, travel,
etc.) holds a different point value. Contractor will be assigned
between 5-10 patients per day that equate to roughly 6-8 points.
Types of cases include Start of Care, Wound Care, Follow-up Visits,
etc. First-timers will not be accepted; recent specialty experience
is required. Weekend requirements vary by location and profession.
On-call requirements vary by location and profession. Territory
mileage radius must be willing to travel up to 75 miles from home
clinic. Local candidates are accepted at the same rate. Travelers
must be open to on-call rotations. Must be comfortable driving to
patients' homes daily and to the clinic for weekly meetings.
Documentation must be completed within 24 hours of patient
appointments. There are no guaranteed hours with this contract.
Weekly productivity points of 30 are expected which equate to 40
hours per week. Delayed and incomplete documentation impacting
billing and follow-up patient care will result in cancellation. The
contractor must accept every patient on their schedule regardless
