Schedule: 3 12-hour shifts each week including alternating weekends What We offer: Great culture and team atmosphere Comprehensive benefits, including medical, dental, and vision, effective on the first of the month 401(k) retirement plan with a generous company match Generous time off accruals Paid holidays Mileage reimbursement Tuition Reimbursement Employee Referral Program Hospice Certification benefit and salary increase Merit Increases Employee Discount Programs What You’ll Do: Provide direct patient care as defined in State Nurse Practice Act Implement current nursing practice following a comprehensive assessment and the plan of care Provide accurate and timely documentation of patient services to reflect the plan of care Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care Participate in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group. Provide appropriate pain/symptom management. Evaluates the patient’s response to treatments/medications Qualifications: Graduate of an accredited practical nurse or a vocational nursing program Three years of nursing experience.
