qualified Home Health RN in Louisville, Kentucky, 40272! Pay
Information $2,252 per week About The Position Job Title:
Registered Nurse I Profession: Nursing Specialty: Home Health
Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 -
17:00) Experience: Minimum 1 full year Adult Home Health Experience
or 1 full year Hospice experience License: Must have license in
hand Certifications: CPR, BLS Must-Have: Proficient in HomeCare
HomeBase and Oasis Description: Provides coordinates and directs
the provision of home nursing care according to physician orders
based on agency policies and procedures. Responsible to Clinical
Manager. Directly supervises and evaluates Home Health Aides and
Licensed Practical Nurses. Home visits and periodic office meetings
are required. Must have reliable transportation and a current
driver's license. Must possess agency-required liability insurance.
Average number of patients per day is 5-8. Contractor will be
assigned between 5-10 patients per day. Each type of visit and
activity holds a different point value, with a total expectation of
30 points per week. Types of cases include Start of Care, Wound
Care, and Follow-up Visits. Must be willing to travel up to 75
miles from the home clinic. Mileage will be reimbursed at the IRS
standard rate. Must be proficient in HomeCare HomeBase and have
used it within the last 6 months. All documentation must be
completed within 24 hours of the appointment with the patient.
Confirmed understanding that there are no guaranteed hours with
this contract. Must accept every patient on the schedule regardless
of distance between appointments. Flexibility with on-call rotation
is required. Must be open to local commuting or traveling for home
