for one another the same way. This is a Full-Time position that
will help the growth of our new hospice division. What You Will Do
as a Hospice RN Field Nurse At Dignity Care Hospice, a Hospice RN

Hospice RN Field Nurse, you will provide support to patients and
families to ensure a safe and comfortable environment. Some of your
primary responsibilities will include: - Completing an initial
assessment of the patient and family to determine hospice care
needs. Providing a complete physical assessment and history of
current and previous illness - Developing a care plan, establishing
goals based on nursing diagnosis and incorporating therapeutic,
preventive, and rehabilitative actions - Re-evaluating and revising
patient care needs as needed - Administering medications and
treatment safely and as prescribed by the physician - Counseling
and educating patients and families in meeting nursing and related
needs - Providing health care instructions to the patient as
appropriate per assessment and plan of care - Participating in

PRN and FT candidates. Education, Licensure & Certification
Requirements - Graduate of an accredited school of nursing - Active
RN license - One (1) to two (2) years of recent acute care
experience - Bachelor’s degree with one (1) year of hospice/home
health experience preferred
