Position Summary:
Under the direction of the Prenatal Outreach Supervisor, the
Community Health Advocate I/II is responsible for conducting
regularly scheduled home visits, educating parents on prenatal and
postpartum care, along with information/education on toddler health
and care through age one, within the ethnic communities of
Montgomery County.
Responsibilities include but not limited to:
Responsible for Case Management, home visiting, documentation, and
care coordination activities including: planning for and providing
face to face home visits, prenatal and postpartum health education,
infant growth and development education with each client within
your caseload; providing appropriate referral and follow up
services for clients, advocating for clients, and functioning as a
community liaison between clients and community agencies to ensure
clients' needs are met
Completing Intake Reports, Head Start Family Strengths and Needs
Assessments, Birth Outcome Reports, Sleep Assessments, Relationship
Assessments, Edinburgh Postnatal Assessments, Sleep Safe
Screenings, and various ages and stages questionnaires, following
up with education and/or referrals based on screening results
Maintaining caseload; canvasing to recruit potential clients
Ensuring attendance at training and quality assurance activities as
needed
Updating and maintaining skills and information
Minimum Qualifications:
Bachelor's degree in Social Science or related field
2-3 years of community-based work experience in community health,
community outreach, or social services
Community Health Advocate I: Initiation/completion of Community
Health Worker (CHW) educational requirements within one year of
employment
Community Health Advocate II: Community Health Worker Certification
(CHW)
Proficient with Microsoft Word, Excel, and PowerPoint
Understands and acknowledges that Public Health - Dayton &
Montgomery County uses evidence-based and evidence-informed public
health practices based on scientific research including health
promotion theory and practice, environmental health measures, and
communicable disease control measures including vaccination,
isolation, and quarantine practices, and medicine for treatment and
prevention of disease, among others
Understands and acknowledges the use of tobacco or nicotine
products in any form including replacement therapy products and
prescription medications used to treat smoking addiction for anyone
hired after January 1, 2014, is prohibited
Required to play an active role during a public health emergency,
crisis, outbreak, incident, or event in which the Health
Commissioner deems additional personnel resources are warranted.
This may include changes in responsibilities and working
locations/hours
Completion of Incident Command System 100 and National Incident
Management System 700 courses within 90 days of hire
Valid State of Ohio driver's license and insurance on any personal
vehicle that will be used for work
Preferred Qualifications:
Community Health Worker Certification (CHW)
Prenatal Healthcare experience
Bilingual
