is currently seeking a Case Manager in the Specialized Recovery
Services (SRS) and Ohio Home Care (OHC) Programs to conduct
eligibility evaluations for individuals applying for specific
programs and creates a person-centered service plan. He/She will
assist the participant with access to and coordination of services
to enable the individual to receive a range of appropriate services
in a planned, coordinated, efficient and effective manner; promotes
and supports the participant’s preferences, values, and the right
to self-determination.
